Relatives of the deceased are required to bring along the originals of the following documents and register the death within 24 hours at the Joint Office
 
  • identity card of the applicant;
  • identity card of the deceased;
  • “Medical Certificate of the Cause of Death” [Form 18]; and
  • “Medical Certificate (Cremation)” [Form 2] (applicable to cremation only).
 
With the above documents in hand, relatives can apply for cremation at the Joint Office or apply for burial at the respective cemetery office.
